The CARL-MOBIL in the rescue chain of Freiburg

When an emergency call is received at the integrated control center in Freiburg and a cardiac arrest, a secured resuscitation or a drowning accident is reported, the CARL-Mobil is alerted directly in addition to an ambulance and an emergency medical service vehicle. The concept of parallel alerting thus saves valuable minutes and shortens the time until the arrival of the treatment team.

A look inside the CARL-Mobil

The CARL-Mobil in Freiburg is an emergency vehicle specialized in extracorporeal resuscitation. On board are two doctors, a perfusionist, an emergency paramedic, and CARL. The aim is to use CARL Technology to increase the chances of survival for people who suffer a cardiac arrest and to prevent neurological sequelae.

The CARL-Mobil Team is alerted from the daily hospital routine and meets directly in front of the University Emergency Center of the Medical Center – University of Freiburg. On average, it takes about 12 minutes after an alert to arrive at the scene in the Freiburg city area. In addition to the CARL-Mobil Team, emergency paramedics, paramedics and an emergency physician are also on site during a resuscitation. Upon arrival at the scene, the CARL Team and the attending emergency physician decide whether extracorporeal resuscitation is indicated based on the information available about the patient and the overall situation. The CARL Team is trained in the use of CARL Technology to provide the best possible CARL therapy. CARL is safely stowed in the ambulance: it is positioned on the CARL Rack, which is attached to the foot end of the patient stretcher.

When the patient’s condition is sufficiently stabilized, he is transferred to Freiburg University Hospital. There, further treatment takes place either directly in the intensive care unit, in the shock room or in the cardiac catheterization laboratory, depending on the suspected cause of the circulatory arrest. In order to optimize the acute in-hospital care of resuscitated patients in the Cardiac Arrest Center, an interdisciplinary reception team, the Cardiac Arrest Receiving Team (CART), was formed at Medical Center – University of Freiburg. This team is composed of staff from intensive care medicine and acute and emergency medicine and coordinates the transfer of the patient to the in-hospital care chain.
